I hereby consent to the above child participating in Water1st’s Youth Programs. I have provided contact details and confirm that all details are correct and I am able to give parental consent for my child to participate Water1st youth activities.

I give permission for my child to volunteer for fundraising and public events, outside of youth advocate meetings, with Water1st staff. I understand that my child’s participation is optional. I understand that at some events guests may purchase or consume alcohol, but my child will NOT be serving or handling alcohol. I understand that it is my child’s responsibility to arrange and coordinate transportation to and from each event.

I give permission to Water1st to record or photograph my child. Water1st may use my child’s image or likeness for publications, social media posts, videos, posters, or other forms of media. This includes interview statements made by my child or work or writing samples submitted by my child. I understand that participation is voluntary and I nor my child expect compensation.hoice
